Tax Form Information

To ensure that you are taxed at the appropriate rate, please note:

• If the school district you will be assigned to lies within a city that collects local taxes, you are required to fill out a tax form for that city

It is imperative that you determine and complete the tax forms applicable to you. Failure to complete a required tax form will result in an automatic filing at the maximum rate with 0 (zero) dependents.
Once the form is completed, please fax the completed form to METS Human Resources at (517) 647-7572 for processing.
